Job summary

ADAPT Community Network in New York City seeks an Assessment Specialist to conduct Home Enabling Supports (HES) assessments and develop person-centered HES Plans for individuals with intellectual and developmental disabilities. The role focuses on identifying technology-based solutions to promote independence, safety, and community participation, while ensuring required documentation meets OPWDD standards.

Assessments may occur in the individual's home or remotely.

Qualifications

• At least 1 year of experience conducting technology assessments.
• Credential in enabling or assistive technology (ETIS/ATP) or clinical background with related disability work.
• Experience with person-centered planning and service delivery is preferred.

Responsibilities

• Conduct comprehensive HES assessments to identify needs and opportunities for technology-based solutions.
• Develop person-centered HES Plans based on assessments and individual goals.
• Evaluate and recommend appropriate assistive technologies.
• Collaborate with individuals, families, Care Managers, vendors, and stakeholders.
• Ensure documentation complies with OPWDD requirements and program standards.
• Maintain accurate records and timely documentation.
