Job summary
The Jewish Board is seeking an Habilitation Support Professional in New York to assist individuals with intellectual and developmental disabilities in home, day habilitation, and community settings.
You will follow individualized plans, promote independence, document progress, and work with families and treatment teams. Strong communication, empathy, and teamwork are essential, with AMAP certification required during probation.
Qualifications
- High School diploma or a minimum of 1 year experience working with the IDD population.
- AMAP certification required for Day Habilitation during probation.
- Basic computer skills with agency software and EHR.
Responsibilities
- Ensure client safety in home/day habilitation settings and promote independence.
- Assist with personal hygiene needs and daily activities.
- Collaborate with guardians/advocates and treatment teams on outcomes.
- Document observations and update ISP/Day Habilitation plans daily.
- Attend treatment planning meetings and support client goals for six months.
- May dispense medications during Day Habilitation hours if needed.
