Job summary

New York Families for Autistic Children seeks a Community Habilitation Worker to provide person-centered supports to individuals with developmental disabilities in home and community settings.

You will implement Habilitation Plans, document services, and promote independence and community integration. The role requires communication skills, confidentiality, and the ability to work as part of an interdisciplinary team in a NY-based environment.

Qualifications

• Good oral and written communication skills.
• Ability to read, write, speak and understand English.
• Ability to maintain confidentiality.
• Strong interpersonal, listening, communication and motivational skills.
• Good analytical ability with problem-solving and follow up.
• Ability to work independently and handle multiple tasks simultaneously.
• Proficiency in the use of a personal computer and appropriate software and E-Mail system.

Responsibilities

• Provide services to persons served in home and community according to their Habilitation Plan.
• Document Habilitation services contemporaneously in accordance with requirements.
• Input into Staff Action Plan in line with Life Plan.
• Monitor safety and welfare, notify supervisor of concerns.
• Communicate health care concerns to supervisor when necessary.
