Job summary

Fresenius Medical Care is seeking an entry-level Home Therapies RN CAP 1 to train and educate dialysis patients on peritoneal and home hemodialysis using our training programs and equipment. The role is clinic-based with initial home visits alongside a staff member.

You will coordinate care, assess patient responses to training, participate in the End Stage Kidney Disease team, and support transplant and home dialysis education, while completing CAP requirements for advancement.

Qualifications

• RN entry level with less than 2 years of nephrology nursing experience.
• Minimum of 1 year RN experience (preferred).
• Home dialysis therapy experience (preferred).

Responsibilities

• Performs essential functions under supervision with guidance from the Educator/Preceptor or RN in charge.
• Collects and analyzes dialysis data, adjusts treatment plans as indicated, documents in EMR and notifies supervisor/physician.
• Educates patient and family on learning needs and supports initial home dialysis training.
• Directs safe delivery of dialysis therapy in collaboration with home care partner and care team.
• Ensures proper lab collection, processing and documentation; reschedules missed samples.
