Qualifications

• Home Therapy nurse trains home hemo or peritoneal dialysis patients.
• In-center hemodialysis, critical care, and home health backgrounds welcome.
• Must be willing to participate in on-call duties.
• Compliance with federal and state regulations.

Responsibilities

• Train patients and care partners in self-care peritoneal dialysis or hemodialysis.
• Conduct home visits to assess home environment and patient needs.
• Document nursing services in the EMR and ensure continuity of care.
• Communicate results to physicians and team members as appropriate.
• Maintain infection control monitoring and emergency preparedness.
• Order and manage clinic supplies to ensure timely patient care and avoid fees.
• Provide on-call nursing services, nights and weekends as needed.
• Assist in data collection for quality improvement activities and patient outcomes.
