Job summary

Fresenius Medical Care seeks an experienced Home Therapies RN CAP 2 to oversee and coordinate home dialysis care for an assigned patient group. You will educate patients and families, lead care teams, and participate in quality improvement within the End Stage Kidney Disease program.

Responsibilities include assessing needs, planning interventions, and teaching RNs, LVNs/LPNs and care staff. The role emphasizes safety, documentation, and collaboration with physicians and supervisors to optimize

Qualifications

• Graduate of an accredited School of Nursing.
• Current state RN licensure.
• CPR/BLS certification required.
• Experience in nephrology or home dialysis preferred.

Responsibilities

• Provide dialysis care to home patients and ensure safety and adherence to treatment plans.
• Coordinate and deliver patient and caregiver education for home therapies.
• Train Home Partners and other staff on equipment and procedures for home dialysis.
• Contribute to quality improvement initiatives and ensure compliance with policy and regulations.
