Job summary

Fresenius Medical Care seeks an experienced RN to serve as an onsite Patient Educator and Case Manager for home dialysis therapies. You will train patients and families, coordinate with the clinic team, and support quality care through the Home Therapies program.

The role includes home visits with another staff member, ensuring safety, educating about ESRD and CKD options, and participating in CQI activities. Travel between facilities may be required.

Qualifications

• RN licensed to practice nursing in the United States.
• Experience in dialysis or home therapies preferred.
• Ability to train patients and families.

Responsibilities

• Provides safe and effective training and oversight of home dialysis therapies.
• Rotates with other licensed staff to ensure adequate coverage.
• Participates in staff training and orientation of new staff as assigned.
• Goes to patient homes to educate on home therapies and equipment.
• Maintains documentation in the Medical Information System and follows policy.
