Job summary

Fresenius Group seeks an entry-level Home Therapies RN CAP participant to educate and coordinate care for home dialysis patients and partners. The role emphasizes training, assessment, and collaboration within the ESKD care team.

The position requires RN licensure, CPR/BLS, and a willingness to work in clinic and patient-home settings. Training provided; on-site work with occasional travel.

Qualifications

• RN with current state licensure and CPR/BLS certification.
• Entry level for nephrology nursing (RN) with <2 years experience; 1 year preferred.
• Home dialysis therapy experience preferred.

Responsibilities

• Provide education and coordinate care for home dialysis patients and partners.
• Collect and analyze dialysis data; update care plan and notify physician as needed.
• Administer medications per protocol and support emergency response measures.
• Train patients and Home Partners on safe operation of home dialysis equipment.
• Collaborate with care team to ensure quality outcomes and program goals.
