Job summary

Fresenius Medical Care is seeking an entry-level Registered Nurse for the Home Therapies CAP to provide and coordinate clinically competent care for home dialysis patients. You will assess, plan, intervene, and evaluate patient care under supervision, and collaborate with physicians and the health care team.

You will train patients and Home Partners, document care plans, manage medications per protocol, and participate in quality improvement activities while complying with policy and regulatory

Qualifications

• Graduate of an accredited School of Nursing.
• Current appropriate state licensure.
• Current or successful CPR BLS Certification.
• Entry level for RNs with less than 2 years of Nephrology Nursing experience.
• Minimum of 1-year RN experience (preferred).

Responsibilities

• Assess and coordinate care for home dialysis patients with the care team.
• Train Home Dialysis patients and Home Partners on equipment and procedures.
• Administer medications per approved protocol and document effectiveness.
• Document patient data and updates in medical records; adjust care plans as needed.
• Participate in CAP activities, staff education, and quality improvement initiatives.
