Job summary

Fresenius Medical Care in Georgia seeks an entry-level Home Therapies RN CAP 1 to provide and coordinate clinically competent care for an assigned group of patients, including assessment, planning, intervention and evaluation within the CAP.

You will educate patients and families, collaborate with the health care team, manage home dialysis training, and participate in quality improvement activities to enhance patient outcomes and facility operations.

Responsibilities

• Performs duties under the direction of the Supervisor with guidance from the Educator, Preceptor or in collaboration with another Registered Nurse.
• Performs ongoing collection and analysis of dialysis data for assigned patients and documents in the patient medical record, makes adjustments to treatment plan as indicated and notifies Supervisor or physician as needed.
• Assesses, collaborates and documents patient/family's learning needs to provide initial and ongoing education to patients and family.
• Directs and provides home dialysis care in collaboration with the patient, home care partner and staff for safe and effective delivery of dialysis therapy to assigned patients.
