Job summary

Fresenius Medical Care in Oregon seeks a Patient Care Technician II Home Therapies to support home hemodialysis and peritoneal dialysis training under the supervision of the HT RN. The role includes patient intake, data collection, and coordination with the care team to ensure safe, effective home therapies.

The position requires a High School diploma or GED, valid PCT certification, and at least two years of patient care experience. Travel between facilities and strong teamwork are expected.

Qualifications

• High School diploma or G.E.D. required.
• Must have a valid PCT national certification or state-specific CMS certification.
• All appropriate state licensure, education, and training required.
• Demonstrated commitment to organization culture, values, and customer service standards.
• Two years of patient care experience preferred.

Responsibilities

• Reinforces home hemodialysis (HHD) and/or peritoneal dialysis (PD) training as permitted by law.
• Identifies and welcomes patients, collects vital signs and weights for intake forms.
• Builds communication with patients, HT RN, staff, and providers.
• Obtains treatment records and reports missing data to the HT RN.
• Assists with home visits and home therapy-related tasks.
• Prepares supplies and manages testing specimens for home therapies.
• Orders and tracks clinic/office supplies as directed.
• Participates in recruitment and orientation of home therapies staff.
