Job summary

Fresenius Medical Care in Oregon seeks a Patient Care Technician II – Home Therapies to support safe and effective home dialysis training under the supervision of a licensed nurse. You will assist in setting up home dialysis equipment and maintain a clean, compliant home therapy environment.

Role involves coordinating with HT RNs and care teams, participating in CQI activities, and contributing to Quality Enhancement Goals through patient satisfaction and process improvement initiatives.

Qualifications

• High school diploma or GED required.
• Valid PCT national certification or CMS state-specific certification required.
• Two years of patient care experience preferred.

Responsibilities

• Reinforces home hemodialysis and peritoneal dialysis training as permitted by law.
• Identifies and welcomes patients, obtains vital signs and weights, and gathers intake information.
• Maintains communication with HT RN, staff, providers, and the care team.
• Obtains treatment records and reports missing or abnormal data to the HT RN.
• Assists with home treatment reconciliation and documentation in medical records.
• Sets up home dialysis training and prepares supplies for visits.
• Performs opening/closing duties and documents activities.
• Disinfects equipment between patient visits and maintains a clean treatment space.
• Assists with scheduling, patient registration, and use of connected health tools.
• Participates in audits, huddles, and process improvement activities.
