Job summary

Fresenius Medical Care is seeking a Patient Care Technician to join our Home Therapies team. The role supports home hemodialysis and peritoneal dialysis training under direct supervision, assisting with equipment setup, room preparation, and patient coordination.

The ideal candidate will have a High School diploma or GED, a valid PCT certification, CPR, and at least two years of patient care experience. Travel between facilities may be required.

Qualifications

• High School diploma or GED required.
• Valid PCT national certification or state-specific CMS certification.
• Two years patient care experience preferred.
• CPR certification required.

Responsibilities

• Reinforces home hemodialysis (HHD) or peritoneal dialysis (PD) training as permitted by law and supports clinical staff.
• Gathers vital signs, weights, and intake information; maintains patient communications.
• Assists with setup/tear-down of home dialysis equipment and ensures a clean, safe home environment.
• Maintains supplies, tracks laboratory specimens, and assists with order entry under supervision.
• Coordinates patient appointments and supports connected health initiatives.
• Participates in staff huddles and contributes to CQI activities and process improvements.
