Job summary

Fresenius Medical Care is seeking a Home Therapy Patient Care Technician to support home dialysis training and treatment under the supervision of a licensed nurse. The role emphasizes safety, cleanliness, and coordination with the HT RN and care partners in a home setting.

The PCT will reinforce HHD/PD training, assist with setup and maintenance of equipment, and participate in QA and infection control processes per CMS and policy guidelines. Strong communication and documentation are essential.

Qualifications

• High School diploma or GED is required.
• Must have a valid PCT national certification or state CMS certification.
• All appropriate state licensure, education, and training required.
• At least two years of previous patient care experience preferred.

Responsibilities

• Reinforces home hemodialysis (HHD) and/or peritoneal dialysis (PD) training as permitted by law.
• Identifies and greets patients, collects vitals, weights, and intake information.
• Maintains communication with HT RN, staff, providers, and the care team.
• Assists with supply prep, lab sample collection, and specimen shipping as directed.
• Schedules appointments and assists patient registration and connected health usage.
