Job summary

Woodhaven is seeking a Home Coordinator II to oversee two residential locations, supervising staff and coordinating day‑to‑day operations for 3 or more individuals served. The role requires self‑motivation, initiative, and independent decision making to ensure compliance with DMH and CARF standards.

Responsibilities include emergency procedure coordination, IP implementation, and active participation on organizational committees.

Qualifications

• High school diploma or equivalent required; bachelor’s in human services preferred.
• Minimum five years experience working with persons with developmental disabilities preferred.
• Must complete required orientation and ongoing trainings (CPR, First Aid, PBIS, universal precautions).

Responsibilities

• Supervise staff and manage two residential locations with 3+ individuals served.
• Coordinate emergency procedures and ensure IPs are implemented appropriately.
• Ensure compliance with DMH certification and CARF accreditation standards.
• Attend to scheduling, coverage, and 24/7 on-call responsibilities.
