Job summary

Rehab Without Walls Neuro Rehabilitation is seeking an experienced Physical Therapist to deliver functional rehabilitation in home, school, work, or community settings. You will evaluate patients, develop personalized treatment plans, and adapt therapies to real-life environments such as kitchens and grocery stores.

Qualifications

• Bachelor’s degree in Physical Therapy from an accredited program.
• Current, unrestricted license as Physical Therapist in the practicing state.
• BLS/CPR certification compliant with state regulations.
• At least one year of physical therapy experience preferred.
• Knowledge of rehabilitation techniques related to neurological injury.
• Effective verbal and written communication.
• Ability to lift up to 100 pounds.
• Standing or walking for extended periods is required.

Responsibilities

• Perform evaluations and develop treatment plans.
• Regularly reassess effectiveness of treatment plans, attend staffing meetings and family conferences.
• Perform discharge evaluations and provide home/community safety recommendations and resources.
• Communicate patient needs and progress to the treatment team, physician, patient and family.
• Collaborate with other disciplines to ensure coordination of care and better patient outcomes.
