Job summary

Rehab Without Walls Neuro Rehabilitation is seeking an experienced physical therapist to provide functional rehabilitation in home and community settings, including homes, schools, workplaces, and community venues.

You will work autonomously within a collaborative team of therapists, delivering evaluations, treatment plans, and progress updates to families and interdisciplinary teams. The role emphasizes real-life functional goals and mobility across various environments.

Qualifications

• Bachelor’s degree in Physical Therapy from an accredited program.
• Current, unrestricted Physical Therapist license in the practicing state.
• Current Basic Life Support (BLS) or CPR certification per state regulations.
• At least 1 year of PT work experience preferred.
• Knowledge of rehabilitation techniques for complex neurological injury.
• Ability to lift 50 pounds and move patients up to 100 pounds following safety procedures.

Responsibilities

• Perform evaluations and develop treatment plans
• Regularly re-assess effectiveness of treatment plans, attend staffing meetings and family conferences
• Perform discharge evaluations, make appropriate recommendations for home and community safety, and provide referral and education resources
• Communicate patient’s needs and progress to the treatment team, physician, person receiving services and family members
• Communicates with other disciplines to ensure collaboration, coordination of care and enhance patient outcomes
