Job summary

BrightSpring Health Services, through Rehab Without Walls Neuro Rehabilitation, seeks a Physical Therapist to deliver functional rehabilitation in home and community settings, helping patients reach real-life goals.

You will assess, plan, implement, and re-evaluate therapy programs, working with families and the treatment team to enhance daily living skills and independence. This role emphasizes autonomy within a collaborative therapist network.

Qualifications

• Bachelor's degree in Physical Therapy and active state license.
• Current Basic Life Support or CPR certification.
• Minimum of one year of physical therapy experience preferred.

Responsibilities

• Perform evaluations and develop treatment plans.
• Regularly reassess effectiveness of treatment plans, attend staffing meetings and family conferences.
• Perform discharge evaluations and provide home/community safety recommendations and referrals.
• Communicate patient's needs and progress to the treatment team, physicians, patients and families.
• Collaborate with other disciplines to ensure coordinated care and improve patient outcomes.
