Elara Caring is seeking a Care Team Supervisor to lead a team of caregivers for Personal Care Services in Nacogdoches County, TX. The role focuses on coordinating services, maintaining records, and ensuring compliance with regulatory standards to support home-based care.

The ideal candidate will have strong leadership, documentation, and communication skills, with experience in fast-paced healthcare settings and familiarity with Medicare/Medicaid processes.

Qualifications

• High School Diploma or GED required.
• 60 college credit hours and/or 2 years of supervisory experience.
• Associate’s or Bachelor’s degree preferred.
• Healthcare experience with elderly or disabled populations preferred.
• 5+ years in fast-paced office or healthcare environment preferred.
• Medicare/Medicaid knowledge preferred.

Responsabilités

• Coordinate and schedule personal care services, including coverage for planned and unplanned absences.
• Maintain accurate client service records, plans of care, caregiver schedules, and required documentation.
• Review and manage service authorizations and contract-specific requirements.
• Monitor reports to ensure accuracy in billing, payroll, utilization, and service delivery.
• Support branch growth initiatives and operational planning efforts.
• Ensure ongoing compliance with licensure, payer, contract, and regulatory standards.
• Provide guidance, coaching, and support to caregivers to promote performance and engagement.
• Ensure caregiver trainings and health assessments are up to date.
• Conduct caregiver orientations as required.
• Collaborate with payroll and timekeeping teams.
• Monitor daily service delivery for adequate caregiver availability.
• Communicate professionally with caregivers, clients, case managers, and leadership.
• Represent Elara Caring positively within the community.
