Job summary

Elara Caring is seeking a Care Team Supervisor for Personal Care Services to lead a team delivering home-based care across Nacogdoches County, TX. This on-site role involves scheduling, documentation, and ensuring compliance with standards while supporting branch growth and operations.

You will coach caregivers, verify trainings and certifications, and partner with payroll. The position requires strong communication, organizational skills, and the ability to travel locally as needed.

Qualifications

• High School Diploma or GED required.
• 60 college credit hours required and/or two years supervisory experience.
• Associate’s or Bachelor’s degree preferred.
• Experience with elderly or disabled populations preferred.
• Five years in a fast-paced office or healthcare environment preferred.
• Working knowledge of Medicare/Medicaid and home care operations preferred.
• Strong documentation skills and organizational abilities.

Responsibilities

• Coordinate and schedule personal care services, including coverage for planned and unplanned absences.
• Maintain client service records, plans of care, caregiver schedules, and required documentation.
• Review and manage service authorizations and contract-specific requirements.
• Monitor billing, payroll, utilization, and service delivery reports.
• Support branch growth initiatives and operational planning.
• Ensure compliance with licensure, payer, contract, and regulatory standards.
• Provide coaching and guidance to caregivers to promote performance.
• Ensure caregiver trainings, health assessments, and in-services are completed.
• Collaborate with payroll and timekeeping teams.
• Monitor daily service delivery and caregiver availability.
• Communicate with clients, case managers, and leadership professionally.
• Represent Elara Caring in the community.
• Perform additional duties to support team and organizational success.
