Job summary

Elara Caring is seeking a Care Team Supervisor for on-site coverage in Nacogdoches County, TX. You will lead a field team of caregivers, coordinate scheduling for both planned and unplanned absences, and ensure documentation and client plans are accurate and up to date.

The role emphasizes leadership, compliance, and collaboration with payroll, case managers, and leadership to maintain quality care and regulatory standards.

Qualifications

• High School Diploma or GED required.
• Sixty college credit hours required and/or two years of supervisory experience.
• Associate’s or Bachelor’s degree preferred.
• Experience with elderly or disabled populations preferred.
• Five years in a fast-paced office or healthcare environment preferred.
• Working knowledge of Medicare/Medicaid and home care operations preferred.
• Strong computer proficiency and documentation skills.
• Excellent organizational and multitasking abilities.
• Professional verbal and written communication skills.
• Strong leadership, coaching, and motivational skills.
• Ability to travel locally and support audits or operational needs.

Responsibilities

• Coordinate and schedule personal care services, including coverage for planned and unplanned absences.
• Maintain accurate client records, plans of care, caregiver schedules, and required documentation.
• Review service authorizations and contract-specific requirements.
• Monitor billing, payroll, utilization, and service delivery reports.
• Support branch growth initiatives and operational planning.
• Ensure compliance with licensure, payer, contract, and regulatory standards.
• Provide guidance and coaching to caregivers.
• Ensure caregiver trainings and certifications are current.
• Conduct caregiver orientations as required.
• Collaborate with payroll/timekeeping teams.
• Monitor service delivery and authorization alignment.
