Home Care Supervisor RN/Admissions RN - Part-Time

Position Overview

Capital City Nurses, a Care Advantage Inc. company, is seeking a PRN Home Care Supervisor RN/Admissions RN to join our team and contribute to our mission of delivering exceptional home care services throughout the greater Maryland and DC area. This position will be based out of our Washington DC office, supporting clients in DC and surrounding areas. We are looking for an exceptionally motivated and passionate Registered Nurse. Key Responsibilities
  • Make initial contact with patients and explain services offered
  • Complete start of care assessments and documentation for onboarding new patients
  • Design individualized care plans for each patient
  • Instruct care team members on patient needs and care plans
  • Report care progress to involved parties
  • Follow up and maintain relationships with clients, advocating on their behalf
  • Establish relationships with community partners and referral sources
  • Keep Clinical Leadership and Office Administrator updated on patient conditions and medical changes
  • Review caregiver documentation
  • Participate in interdisciplinary team meetings to meet patient needs
  • Perform other duties as assigned by the Administrator
Requirements
  • Active DC Registered Nurse license
  • Minimum of 1 year experience in a nursing role (LPN to RN acceptable)
  • Minimum of an Associate Degree in Nursing
  • Ability to work collaboratively in cross-functional teams
  • Effective verbal and written communication skills
  • Excellent presentation skills
  • Reliable transportation for home visits with mileage reimbursement
  • Proficiency with computer/iPad documentation tools
