Role

The Admissions Registered Nurse (RN) will provide and coordinate hospice care for patients and families, collaborating with the interdisciplinary team. Responsibilities include initial assessments, judgments, and treatment decisions based on patient needs and wishes, adhering to professional standards.

Qualifications
  • Bachelor's degree in nursing or related field, or equivalent experience
  • Current RN license (state-specific or multistate/compact)
  • Minimum two years RN experience in acute care, hospice, or home health
  • Strong clinical assessment skills; ICU or emergency background preferred
  • Experience with EMR/EHR systems
  • Effective communication skills in English; bilingual in Spanish preferred
  • Valid driver’s license and reliable transportation
  • Ability to work in a team, manage multiple priorities, and handle emotional stress
  • Physical ability to perform job functions with or without accommodations
  • Active BLS certification from AHA or Red Cross (prior to 12/31/2022, must obtain by 10/1/2023)

Job Responsibilities
  • Complete clinical admissions, explain services, address concerns, and educate families
  • Obtain consents, conduct assessments, and develop care plans
  • Coordinate with physicians and facilitate referrals and discharges
  • Maintain documentation and communication using SBAR
  • Build relationships with healthcare facilities and professionals
  • Provide care for GIP patients in hospitals, including management and documentation
  • Perform additional duties as assigned

Compensation Range

$35.42 - $53.14
