Job summary

Redwood Coast PACE in Eureka, CA seeks a Home Care Supervisor to lead the in-home care team and supervise CarePartners. The role supports advocacy, education, coordination, documentation, and direct participant care from a licensed nursing perspective.

The schedule is Monday–Friday, 8 am–5 pm with occasional on-call. The ideal candidate will have LVN licensure (CA), at least 1 year of supervisory experience, and a strong commitment to elder care in the home setting.

Qualifications

• CA LVN licensure required.
• Minimum 1 year supervisory experience.
• Experience with frail/elderly population preferred.

Responsibilities

• Leads and supports the Home Care team including advocacy, education, coordination, and documentation.
• Directly supervises the in-home CarePartner team and supports scheduling and training.
• Ensures safety, maintains nursing records, and complies with program requirements.
• Coordinates with physicians, families, and outside agencies as needed.
