Job summary

Elara Caring is seeking a Speech Language Pathologist to provide high-quality home-based speech-language pathology services. You will assess patients, develop plans of care, and deliver therapy in patients’ homes, coordinating with physicians and the care team.

The role involves travel up to 50% within the branch coverage area, lifting required, and maintaining thorough clinical notes in line with PPS and OASIS guidelines.

Qualifications

• Master's degree from an accredited program in Language Pathology.
• Current, unrestricted SLP license in the state of work.
• Minimum one year of experience as an SLP in a clinical care setting.
• Able and willing to travel within branch/office coverage area; dependable vehicle, valid driver’s license, and current auto insurance.
• Must be able and willing to travel 50% and lift 50-100 lbs.

Responsibilities

• Promotes Elara Caring's philosophy, mission statement and policies to ensure quality of care.
• Evaluates the patient and develops a plan of care.
• Admits the patient and assesses therapy needs per OASIS and PPS.
• Performs initial and follow-up therapy evaluations and documentation.
• Communicates findings to supervisor and care team; participates in discharge planning.
• Writes physician orders and integrates patient goals into therapy notes.
• Ensures accurate therapy notes and logs are maintained.
