Arcadia Home Care is seeking a Full Time Service Coordinator in Prescott, AZ to schedule and supervise in-home care workers and clients, ensuring care plans are followed and quality is monitored.

You will conduct home visits, report on services, handle coverage, and maintain compliance with Medicare/Medicaid/HIPAA while supporting aging adults and individuals with disabilities to live safely at home. Benefits include medical, 401K, education, and more.

Qualifications

• Minimum of 2 years’ experience in senior services or a related social service field.
• Excellent oral and written communication skills.
• Proficient in Microsoft Word, Excel, and Outlook.
• Must have reliable transportation and a valid driver’s license.

Responsibilities

• Create and manage staff schedules in the scheduling system.
• Provide alternate coverage to ensure care plans are followed and not interrupted.
• Contact care providers and clients for service updates.
• Monitor client condition and Home Care Aides performance.
• Ensure compliance with Medicare, Medicaid, HIPAA regulations and company policies.
• Conduct home visits to new clients and review Welcome Packet.
• Observe and evaluate Home Care Aides in client homes as directed by Supervisor.
• Prepare accurate evaluation reports and case notes.
• Maintain schedules to ensure timely completion of visits.
• Attend weekly branch meetings.
• Maintain confidentiality and attendance.
