Job summary

Ambercare – an Addus family company seeks a Service Coordinator in Anthony, NM to organize caregiver schedules, support field staff, and ensure care plans stay on track. You will be the go-to problem solver, handling client updates and coordinating coverage for interruptions while maintaining confidentiality.

Full-time position with Monday–Friday 8am–5pm, occasional on-call weekends. Benefits include medical/dental/vision, daily pay, PTO, retirement planning and life insurance, plus recognition

Qualifications

Responsibilities

• Creates and manages caregiver schedules in the system.
• Ensures care plans are followed and client services are uninterrupted.
• Contacts providers and clients for service updates and wellbeing calls.
• Conducts home visits as required and supports on-call needs.

We’re looking for a driven, organized, and compassionate Service Coordinator to lead the charge in scheduling caregivers, ensuring top-quality service, and conducting in-home visits. You’ll be the go-to problem solver—juggling schedules, supporting field staff, handling client updates, and stepping in to keep care plans on track. Monday thru Friday - 8am to 5pm - 1 Hour lunch and rotation on call weekends.

Essential Duties:

• Creates work schedules by entering schedules into the system, manages changes to client schedules due to client request, illness, vacation or leaves of absence.
• Provides alternate coverage to ensure the client’s care plan is followed and client services are not interrupted.
• Contacts care providers and clients to provide service updates
• Conducts monthly client wellness calls and conducts home visits as required
• Provides thorough, complete follow-through on escalated client complaints and theft claims
• Supervises direct service employees by setting expectations for attendance, performance and conduct by holding employees accountable to the company’s policies and guidelines
• Ensures the appearance of the branch’s open environment is professional: neat, clean, orderly and generally free of clutter
• Maintains a high degree of confidentiality at all times due to access to sensitive information
• Maintains regular, predictable, consistent attendance and is flexible to meet the needs of the agency
• Follows all MCO, Medicare, Medicaid, and HIPAA regulations and requirements
• Abides by all regulations, policies, procedures and standards

Position Requirements & Competencies:

• Must have high school diploma or equivalent.
• 4 years of office experience
• Interpersonal, organizational and communication skills.
• Computer skills including but not limited to Microsoft Word, Microsoft Excel and Scheduling program.
• Must have reliable transportation and a valid DL for travel to home visits monthly and some marketing
