NHC HomeCare Greenville is seeking a Scheduler to join our team. The Scheduler facilitates patient schedules for multiple disciplines, as collaboratively directed by the Office Manager and Director of Nursing.

Responsibilities include timely data entry, notifying partners and patients of changes, coordinating admissions and discharges, and relaying information between disciplines. LPN license preferred; strong customer service and communication skills required.

Qualifications

• High school diploma or GED required.
• Two years’ experience in home care or related health care role required.
• Computer literacy required.
• LPN license preferred if applicable.

Responsabilités

• Facilitates daily schedules with timely data entry of changes.
• Notifies partners and patients of scheduling updates promptly.
• Coordinates admissions, evaluations, resumption of services, and discharges with DON-directed disciplines.
• Relays information between homecare disciplines about changes and orders.
