Job summary

Elara Caring is seeking a Scheduler to support administrators by managing patient visit schedules and coordinating care team activities. The role requires adapting to busy environments and communicating with clinicians, physicians, and staff to ensure timely, high-quality patient visits.

The ideal candidate will have a high school diploma or GED (associates preferred), 1 year of scheduling experience in home care, and familiarity with Homecare Homebase and Workday.

Qualifications

• High school diploma or GED required; Associate degree preferred.
• 1 year of practical experience required.
• 1 year of patient scheduling in home care preferred.
• Familiar with national standards of care, state licensure, and federal conditions of participation.
• Ability to convey information clearly and to plan ahead.

Responsibilities

• Schedule patient visits and coordinate with care team and CTM.
• Act as liaison between Clinical Team Manager, staff, physicians and facility staff.
• Ensure timely, accurate scheduling to support patient care.
• Assist in resolving scheduling conflicts and patient inquiries.
