Job summary

The role requires at least a high school diploma, 1 year of scheduling experience, and proficiency with enterprise systems (Homecare Homebase, Workday). Bilingual Spanish/English preferred.

Qualifications

• Requires scheduling experience in a home care setting or equivalent.
• Strong communication and interpersonal skills.
• Ability to interpret physician orders and coordinate with clinical team.

Responsibilities

• Schedule patient visits and coordinate with clinicians to ensure timely care.
• Serve as liaison between CTMs, direct care team members, physicians, and facility staff.
• Monitor schedules, resolve conflicts, and document patient care activities.
