Job summary

Elara Caring is seeking a Home Health Scheduler to support Administrators in scheduling patient visits for the nursing team. You will coordinate with the CTM, direct care staff, physicians, and facility staff to ensure the right care is delivered at the right time and place.

The role requires a high school diploma or GED, and at least one year of electronic scheduling experience, preferably in home health. Training in Homecare Homebase/Workday and MS Office is expected.

Qualifications

  • High school diploma or GED is required.
  • 1 year of electronic scheduling experience is required.
  • 1 year of patient scheduling experience in home care setting is strongly preferred.
  • Knowledge of nursing visits is preferred or experience with a nursing team.
  • Familiarity with national standards of care, state licensure, and federal conditions of participation.
  • Ability to convey information clearly, listen, verify understanding, and respond appropriately.
  • Ability to set timeframes, allocate resources, and plan ahead.
  • Proficiency with enterprise applications (Homecare Homebase, Workday, etc.) and MS Office.
  • Ability to handle complaints, manage conflict and negotiate outcomes.
  • Ability to seek patient feedback, advocate for patient needs, and manage expectations.
  • Bilingual in Spanish and English preferred.

Responsibilities

  • Schedule patient visits for the Nursing team and support Administrators or Designated Branch Directors.
  • Serve as an intermediary between the Clinical Team Manager, direct care team, patient’s physician, and facility staff overseeing patient care.
  • Support ongoing delivery of right care at the right time and place by coordinating resources and information.

Job Description: The Home Health Scheduler for the Nursing team supports Administrators (or Designated Branch Directors) in scheduling patient visits. What is Required?
  • High school diploma or GED required
  • 1 year of electronic scheduling experience required
  • 1 year of experience of patient scheduling in the home care agency setting is strongly preferred
  • Knowledge of Nursing visits preferred, or experience working with a nursing team
  • Demonstrates familiarity and understanding of national standards of care, state licensure, and federal conditions of participation.
  • Ability to convey information and opinions, listening to others and checking for understanding, responding in appropriate message and tone
  • Ability to set timeframes, match resources to tasks, and plan ahead
  • Quickly become proficient in enterprise applications (Homecare Homebase, Workday, etc.) and intermediate knowledge of Microsoft Office
  • Ability to handle complaints, manage conflict and negotiate outcomes
  • Ability to seek patient feedback, advocate for patient, understanding of patient needs, and managing patient expectations
  • Bilingual in Spanish and English preferred

You will report to the Administrator or Branch Director.
