Job summary

Charter Oak Home Care is seeking a Care Coordinator to support office operations and the scheduling and matching of clients with caregivers in the Torrington area. The role involves coordinating with clients, families, case managers, and staff, and managing daily caregiver-client schedules to balance needs and availability.

The position includes client intake, onboarding, and occasional home visits to ensure service satisfaction.

Qualifications

• Excellent interpersonal skills and ability to interact with clients, families and staff.
• Strong attention to detail in scheduling and documentation.
• Ability to work in a fast-paced, ever-changing environment and multitask.
• Familiarity with Google Business Suite and scheduling software.
• Reliability and ability to pass background checks and provide references.
• Eligibility to work in the U.S. and ability to communicate effectively.

Responsibilities

• Schedule care for clients in their home.
• Update client care plans and ensure accuracy.
• Communicate with clients, families and caregivers to ensure smooth delivery of care.
• Handle administrative tasks and report changes in client condition.
• Assist with manager-directed projects as needed.
