• Fill open shifts and maintain continuity of care for clients.
• Answer incoming telephone calls and triage client, caregiver, and office needs.
• Maintain scheduling documentation in the agency's software system.
• Enter and track employee time-off requests.
• Distribute client and caregiver schedules as needed.
• Track caregiver credentials and ensure documentation remains current.
• Track required employee education and training requirements.
• Complete filing, data entry, scanning, faxing, and other administrative tasks.
• Communicate professionally with clients, families, caregivers, and office teammates.
• Maintain confidentiality and comply with Nova Leap Home Health policies.
• Participate in rotating after-hours and weekend on-call schedule with stipend.

Home Care Scheduler - Full Time

Nova Leap Home Health | South Deerfield, MA

Monday-Friday | 9:00 AM-4:00 PM | 35 Hours per Week

Nova Leap Home Health is seeking a reliable, highly organized Home Care Scheduler to join our South Deerfield office.

This is an important, fast-paced role responsible for coordinating caregiver schedules and helping ensure our clients receive consistent, dependable care. We are looking for someone who thrives in a busy office environment, can manage multiple priorities at once, communicates exceptionally well, and takes ownership of getting the schedule covered.

Home care or healthcare experience is strongly preferred. Candidates with previous caregiving experience who are ready to transition into an office-based role are also encouraged to apply.

What You’ll Do

The Home Care Scheduler will independently manage the daily caregiver schedule while providing administrative and operational support to the South Deerfield office.

Responsibilities include:

• Independently manage and maintain the daily client and caregiver schedule.
• Coordinate caregiver assignments based on client needs, employee availability, location, and qualifications.
• Respond quickly to caregiver call-outs, late arrivals, and other last-minute schedule changes.
• Work proactively to fill open shifts and maintain continuity of care for clients.
• Answer incoming telephone calls and appropriately triage client, caregiver, and general office needs.
• Maintain accurate scheduling documentation in the agency's software system.
• Enter and track employee time-off requests.
• Distribute client and caregiver schedules as needed.
• Track caregiver credentials and assist in ensuring required documentation remains current.
• Track required employee education and training requirements.
• Complete filing, data entry, scanning, faxing, and other administrative responsibilities.
• Communicate professionally with clients, families, caregivers, and members of the office team.
• Maintain confidentiality and comply with all Nova Leap Home Health policies and procedures.
• Participate in a rotating after-hours and weekend on-call schedule, with an additional stipend provided.

• Experience using home care scheduling software.
• Understanding of the challenges associated with caregiver staffing and maintaining continuity of care.

Schedule

Full-Time - 35 Hours per Week

Monday through Friday, 9:00 AM-4:00 PM

Schedule may be modified based on office and operational needs. This position also participates in a rotating after-hours/weekend on-call schedule with an additional stipend.
