Job summary

Loyola Medicine Home Care and Hospice seeks a Palliative Care Registered Nurse to provide clinical consultation and care for patients and families in the palliative program. The RN collaborates with the care team to improve quality and supports staff in delivering compassionate end-of-life care.

Responsibilities include developing performance measures, guiding staff, and delivering data to support program operations. Requires RN licensure and CPR; hospice experience preferred.

Qualifications

• Graduate of an approved nursing education program.
• Current Registered Nurse licensure in the state of practice.
• CPR certification.
• Two years acute care background; Hospice/Palliative Care certification preferred.
• Home care experience; strong clinical knowledge base.
• Palliative care experience; ability to use problem solving and priority setting.
• Ability to work independently and on a team.
• Ability to develop, coordinate and evaluate palliative/end-of-life care programs.

Position Purpose:

• Collaborates with manager in the development of performance measurements for the Palliative Care Program to ensure continuous quality improvement; ensures implementation
• Motivates/supports employees for exemplary performance on internal and external customer service, productivity, and accuracy
• Provides statistical/budgetary data to appropriate Palliative care program
• Works collaboratively with all departments to ensure execution of quality delivery of services

Minimum Qualifications:

• Graduate of an approved nursing education program.
• Current Registered Nurse licensure in the state of practice.
• CPR certification
• Two years acute background and Hospice/Palliative Care certification preferred.
• Home care experience, strong clinical knowledge base, Hospice experience, computer skills, strong interpersonal and organizational skills,
• Palliative Care experience, demonstrate ability to use problem solving, critical thinking and priority setting skills
• Must be able to work independently and on a team
• Ability to perform diversified duties with time limitations
• Ability to develop, coordinate and evaluate programs in Palliative care and end-of-life care
