Loyola Medicine Home Care and Hospice is seeking a Palliative Care Registered Nurse to support patients and families in the palliative care program. Provides palliative care clinical consultation.

The role emphasizes collaboration with the care team, quality improvement, and compassionate end-of-life care with strong clinical judgment. Day shift, no on-call, and a $10,000 bonus accompany comprehensive day-one benefits.

Qualifications

• Graduate of an approved nursing education program.
• Current Registered Nurse licensure in Illinois.
• CPR certification is required.
• 2-3 years acute care and hospice/palliative care preferred.
• Home care experience and strong interpersonal skills.

Responsibilities

• Collaborates with management to develop performance measures for the palliative care program.
• Provides palliative care clinical consultation and guides quality improvement.
• Supports staff to maintain high-quality service delivery.
• Develops tracking of patient outcomes for review and reporting.
• Demonstrates Trinity Health Mission, Vision, and Values in practice.
