Job summary

Loyola Medicine Home Care and Hospice is seeking a Palliative Care RN to support patients and families in our palliative care program, providing expert clinical consultation and compassionate care.

This full-time position offers day-one medical, dental and vision benefits, a $10,000 bonus, generous paid time off with 7 holidays, and a comprehensive orientation. Illinois RN licensure, CPR, and hospice/palliative care experience are preferred.

Qualifications

• Graduate of an approved nursing education program.
• Current Registered Nurse licensure in the state of practice.
• CPR certification.
• Two years acute background; Hospice/Palliative Care certification preferred.
• Home health/hospice experience and strong interpersonal and organizational skills.
• Palliative Care experience; strong problem solving and priority setting abilities.
• Must be able to work independently and on a team.

Responsibilities

• Develop performance measurements for the Palliative Care Program to ensure continuous quality improvement; ensures implementation.
• Motivate/supports employees for exemplary performance on internal and external customer service, productivity, and accuracy.
• Provides statistical/budgetary data to appropriate Palliative care program.
• Works collaboratively with all departments to ensure execution of quality delivery of services.

What You Will Do

• Collaborates with manager in the development of performance measurements for the Palliative Care Program to ensure continuous quality improvement; ensures implementation
• Motivates/supports employees for exemplary performance on internal and external customer service, productivity, and accuracy
• Provides statistical/budgetary data to appropriate Palliative care program
• Works collaboratively with all departments to ensure execution of quality delivery of services

Minimum Qualifications

• Graduate of an approved nursing education program.
• Current Registered Nurse licensure in the state of practice.
• CPR certification
• Two years acute background and Hospice/Palliative Care certification preferred.
• Home care experience, strong clinical knowledge base, Hospice experience, computer skills, strong interpersonal and organizational skills,
• Palliative Care experience, demonstrate ability to use problem solving, critical thinking and priority setting skills
• Must be able to work independently and on a team
• Ability to perform diversified duties with time limitations
• Ability to develop, coordinate and evaluate programs in Palliative care and end-of-life care
