Job summary

Socket.dev is seeking a Hospice RN Case Manager to deliver compassionate, patient-centered nursing care for hospice patients and families. The role emphasizes assessment, care planning, symptom management, and coordination of services to ensure quality end-of-life care.

You will work with physicians, hospice aides, LPNs, social workers, chaplains and other team members, document care plans, and educate families while providing grief support.

Qualifications

• Graduate of an accredited nursing program.
• Current RN license in the state of practice.
• CPR certification required.
• Minimum one year of acute care nursing experience.
• Hospice or home health experience preferred.
• Strong communication, assessment, and relationship-building skills.
• Valid driver's license and reliable transportation.

Responsibilities

• Manage a hospice patient caseload as primary case manager.
• Complete comprehensive patient assessments and develop individualized care plans.
• Coordinate care with physicians, hospice aides, LPNs, social workers, chaplains, and other interdisciplinary team members.
• Manage pain and symptoms using hospice and palliative care practices.
• Educate patients and families on disease progression and available hospice services.
• Maintain accurate clinical documentation.
• Participate in interdisciplinary team meetings and conferences.
• Supervise Hospice Aides and LPNs, including supervisory visits and care plan oversight.
• Support patients and families through grief, loss, and emotional transitions.

Position Summary
The Hospice RN Case Manager provides compassionate, patient-centered nursing care to hospice patients and families. This role is responsible for assessment, care planning, symptom management, coordination of services, patient and caregiver education, and overall case management to ensure quality end-of-life care.

Key Responsibilities
