Job summary

Trinity Health At Home is seeking a Hospice RN Case Manager to provide compassionate, patient-centered nursing care for hospice patients and families. You will assess, plan, and manage care, coordinate with an interdisciplinary team, and guide patients through end-of-life support.

Responsibilities include comprehensive assessments, care planning, symptom management, education for patients and families, and meticulous documentation.

Qualifications

• Graduate of an accredited nursing program.
• Current RN license in the state of practice.
• CPR certification required.
• Minimum one year of acute care nursing experience.
• Hospice or home health experience preferred.
• Strong communication, assessment, and relationship-building skills.
• Valid driver's license and reliable transportation.

• Coordinate care with physicians, hospice aides, LPNs, social workers, chaplains, and other interdisciplinary team members.

• Maintain accurate and timely clinical documentation.
• Participate in interdisciplinary team meetings and care conferences.
• Supervise Hospice Aides and LPNs, including required supervisory visits and care plan oversight.
• Support patients and families through grief, loss, and emotional transitions.

Position Summary
