Rochester Regional Health is seeking a Registered Nurse for Home Care at ElderOne – Seneca Towers in Rochester, NY. This 40-hour week role runs Monday–Friday 8:00 am–4:30 pm with one weekend per quarter, and involves coordinating 24-hour home care and patient education.

Qualifications

• Nursing Diploma or ASN required.
• Registered RN in NY.
• New York driver's license required.
• BSN preferred.
• 1 year of experience with frail or elderly population preferred.
• Motor vehicle availability required.
• 6 months to 1 year UAS-NY Community Assessment experience preferred.

Responsabilités

• Patient Care: Conduct initial assessments; establish plan of care and provide prescribed treatments; coordinate 24-hour care.
• Communication: Counsel and guide individuals and families towards self-help for physical, emotional, and environmental problems.
• Evaluation & Documentation: Reassess patients regularly; maintain complete and accurate clinical documentation; supervise and support home care staff.
