Job summary

Rochester Regional Health’s Home Care team is seeking a Registered Nurse for ElderONE in Rochester, NY. You’ll conduct assessments, develop plans of care, and deliver prescribed treatments while coordinating 24-hour care with a dedicated team.

You’ll counsel patients and families, guide self-help strategies, and document care with accuracy. A Nursing Diploma and NYS RN license are required; BSN preferred and a valid NYS driver’s license.

Qualifications

• Nursing Diploma required.
• License and current registration to practice as Registered Professional Nurse in New York State.
• Current NYS Driver’s license required.

Responsibilities

• Conduct initial patient assessments and establish plan of care with prescribed treatments.
• Coordinate delivery of 24-hour care with a collaborative home care team.
• Counsel and guide individuals and families toward self-help in health problems.
• Maintain complete and accurate clinical documentation; supervise home care staff as needed.
