Job summary

Rochester Regional Health in Rochester, NY, seeks a Registered Nurse - Home Care to provide compassionate, patient-centered care in clients' homes. You will perform initial assessments, develop plans of care, administer prescribed treatments, and coordinate 24-hour home care delivery.

Qualifications

• License and current registration to practice as Registered Professional Nurse in New York State.
• Current NYS Driver’s license.
• AS: Nursing (Required).
• BSN preferred; 6 months-1 year experience with UAS-NY Community Assessment preferred.

Responsibilities

• Conduct initial assessments; establish plan of care and provide prescribed treatments and demonstrated nursing abilities; coordinate the delivery of 24-hour care.
• Counsel and guide individuals and families towards self-help in recognition and solution of physical, emotional and environmental problems.
• Reassessment of each patient on a regular basis; maintain complete and accurate clinical documentation; supervise and serve as a resource to home care staff.
