Rochester Regional Health is seeking a Registered Nurse for Home Care at ElderOne - Seneca Towers, Rochester, NY. This 40-hour-per-week role offers Monday–Friday 8am–4:30pm with one weekend per quarter, and comprehensive benefits within a cutting-edge PACE program that supports seniors at home.

Responsibilities include patient care, teaching, and coordinating 24-hour care. Required: RN in NY, Nursing Diploma/ASN, NY driver’s license. Preferred BSN and experience with frail or elderly populations.

Qualificações

• Nursing license in NY is required.
• New York RN license is required.
• A valid NY driver's license is required.

Responsabilidades

• Provide patient care: Assess, plan, and implement care and coordinate 24-hour home care delivery.
• Counsel and guide patients and families on self-care and home-based health management.
• Conduct regular reassessments and maintain accurate clinical documentation; support home care staff.
