Rochester Regional Health Home Care in Rochester, NY is seeking a Registered Nurse (RN) for Community Health nursing in the home care setting. You will perform nursing interventions, patient education and care management, delivering one-on-one care in patients’ homes.

The role offers flexible scheduling, mileage reimbursement, shift differentials, opportunities for overtime, and tuition assistance to support BSN to MSN advancement, along with a comprehensive benefits package.

Qualifications

• Diploma or Associate’s Degree in Nursing.
• Registered Nurse license in New York State.
• Valid NYS Driver’s License.
• Bachelor’s Degree in Nursing preferred.
• 1 year of nursing experience preferred.
• Prior home health, clinical and direct patient care experience.

Responsabilités

• Provide comprehensive assessments of bio-psycho-social needs and perform skilled nursing care.
• Develop and document individualized care plans with the interdisciplinary team.
• Communicate patient health status and treatment plans with staff and families.
• Accurately document client care and physician orders per standards.
• Adhere to department and system protocols, regulations, and education requirements.
