Rochester Regional Health is seeking a Registered Nurse (RN) at ElderONE -Seneca Towers in Rochester, NY. The role supports a PACE program delivering coordinated, compassionate in-home and clinic care as part of a collaborative, mission-driven team.

The position offers a per diem schedule (3pm-11pm, Monday-Friday, plus two weekends per month) with shift differentials and robust benefits, including tuition assistance and opportunities for leadership advancement.

Qualifications

• Diploma or Associate’s Degree in Nursing is required.
• Registered Nurse license in New York State is required.
• Basic Life Support (BLS) certification is required.

Responsibilities

• Provide nursing care and support following the nursing process, collaborating with physicians and the care team.
• Develop and document individualized care plans and communicate progress to the team.
• Proficient use of an Electronic Health Record (EHR) for documentation and communication.
• Adhere to department and system protocols, regulations, and education requirements.
