Position Purpose: Are you ready to make a meaningful impact in patients' homes? Join Mount Carmel Home Care as a Primary Care Nurse, utilizing cutting-edge technology and clinical expertise to deliver exceptional care. Specializing in IV and infusion therapy, you'll provide vital services to those in need, along with other essential home care services.

What You Will Do:

• Deliver skilled nursing care with a focus on infusion therapy.
• Develop and implement personalized patient care plans.
• Educate patients and families on managing illnesses at home.
• Coordinate with physicians and other healthcare professionals to ensure comprehensive care.
• Utilize industry-leading EMR systems to document patient care accurately and efficiently.

Minimum Qualifications:

• Current RN license in Ohio.
• At least 2 years of nursing experience, with strong infusion therapy skills.
• Valid driver’s license and reliable transportation.
• Excellent communication and organizational skills.
• Ability to work independently and manage time effectively.
• Preferred experience in Home Care or Med/Surg Hospital settings.
