Role Responsibilities

• Coordinate total care plans with healthcare professionals to ensure continuity of care, within a Point of Care setting.
• Conduct initial and comprehensive nursing evaluations, develop care plans per state guidelines, and document accurately and promptly in collaboration with physicians.
• Perform patient assessments and develop appropriate care plans.
• Verify patients meet home health eligibility and medical necessity criteria as defined by payers.

License and Certification Requirements

• Current RN licensure in the practicing state
• Current CPR certification
• Valid Driver's License, vehicle insurance, and access to reliable transportation
