Qualifications

• Graduate of an accredited School of Physical Therapy.
• Licensed or eligible for licensure to practice in the State of Connecticut.
• Prefer experience with electronic medical charting.

Responsibilities

• Develops a plan of care for each patient with input from the patient.
• Interprets and assesses patient data, sets and revises goals.
• Recommends therapeutic procedures and modalities as appropriate.
• Maintains documentation per standards.
• Addresses a home exercise program with each patient.
• Maintains an adequate caseload and productivity.
• Participates in continuing education and staff education.
• Communicates with Rehabilitation Supervisor and physicians.
