Home Care Rehab in Middletown, CT seeks a Physical Therapist to administer physical therapy services in accordance with prescription, hospital policy and state law. The role includes clinical oversight of the PTA, developing patient care plans, and ensuring delivery of education and treatments to patients and families.

Qualifications

• Graduate of an accredited School of Physical Therapy.
• Licensed or eligible for licensure to practice in the State of Connecticut.
• Experience with electronic medical charting.

Responsabilités

• Develops a plan of care for each patient with input from the patient.
• Interprets and assesses patient data, sets goals, and revises goals as needed.
• Explains treatment goals, plans, techniques and outcomes to patient/family.
• Recommends therapeutic procedures and modalities as appropriate.
• Maintains patient caseload and adheres to productivity standards.
• Attends continuing education and presents to Rehab staff or clinicians.
• Communicates with Rehabilitation Supervisor and referring physicians.
