Home Care Rehab in Middletown, CT is seeking a Physical Therapist to administer physical therapy services under a prescription, with clinical oversight of PTA. Duties include developing individualized plans of care, explaining goals to patients and families, and recommending therapeutic procedures.

Candidate must have PT degree, CT licensure eligibility, and 3-5 years of clinical experience; experience with electronic medical charting is preferred.

Qualifications

• Graduate of an accredited School of Physical Therapy.
• Licensed or eligible for licensure to practice in Connecticut.
• 3-5 years clinical experience.
• Experience with electronic medical charting.

Responsibilities

• Develops a plan of care for each patient with the patient's input and agreement, which includes safety, educational and developmental needs.
• Interprets data and sets goals, explaining treatment plans, techniques and outcomes to patient/family.
• Recommends therapeutic procedures and modalities.
• Maintains documentation per standards.
• Addresses a home exercise program with each patient.
• Maintains an adequate caseload per productivity standards.
• Attends continuing education and presents educational programs.
• Communicates with Rehabilitation Supervisor, team members and referring physicians.
