Job summary

Middletown, CT Home Care Rehab seeks a Physical Therapist to provide therapy services under prescription, in line with hospital standards and the Connecticut Practice Act. The PT will oversee PTA and develop patient-centered care plans, including safety, education, and family communication.

Minimum requirements include graduation from an accredited PT program and CT licensure or eligibility; 3–5 years of clinical experience; preferred EMR experience.

Qualifications

• Graduate of an accredited School of Physical Therapy.
• Licensed or eligible for licensure to practice in the State of Connecticut.
• Experience with electronic medical charting (EMR).
• 3-5 years clinical experience.
• Flexible scheduling.

Responsibilities

• Develops a plan of care for each patient with input from the patient and family.
• Interprets and assesses patient data, sets and revises goals.
• Recommends therapeutic procedures and modalities.
• Maintains an adequate caseload and adheres to productivity standards.
• Communicates with Rehabilitation Supervisor and referring physicians.
• Attends continuing education seminars and presents to Rehab staff or other professionals.
• Performs all other job-related duties as assigned.
